Active Living with Chronic Conditions
Active Living with Chronic Conditions is an online interactive program for people experiencing any type of chronic disease and their caregivers. Participants learn to live a healthier life with issues related to chronic conditions. The program is a condensed version of the Chronic Disease Self-Management Program.
Description
Managing a chronic health condition can be difficult, but support is available. Active Living with Chronic Conditions is an evidence-based program that empowers adults to take charge of their health, build confidence, and enhance their quality of life.
The program is designed for adults with various chronic health conditions. Caregivers, family members, and friends are also encouraged to join.
Over six (6) interactive sessions, participants will gain practical tools and strategies to:
- Manage the physical and emotional challenges that can come with chronic conditions, including fatigue, pain, stress, and frustration
- Communicate effectively with healthcare providers, family, and friends
- Develop healthier eating and physical activity habits
- Set realistic, achievable goals and create action plans
- Build confidence in managing their health and everyday activities
Participants can connect with others facing similar challenges while learning proven strategies for a healthier, more active, and fulfilling life.
Important Registration Information:
- Sessions are offered virtually through Microsoft Teams or by phone.
- Registration is required for each participant, and space is limited.
- Microsoft Teams calendar invitations will be sent approximately one week before the first class.
- Please do not forward the Microsoft Teams invitation. Each participant must register individually through the TGH website.
- Registration closes two days before the first class.
- The program requires a minimum number of registered participants to proceed.
- If enrollment does not meet the minimum, the program will be canceled, and registered participants will be notified 24 hours before the scheduled start date and time.
Technology requirements: Internet access, computer, laptop, tablet or smartphone with video, microphone, and speaker capabilities.
