Blood Pressure Self-Monitoring Program
If you are living with high blood pressure, taking action to keep it well-controlled will minimize your risk for complications like stroke, heart attack, and even death. TGH has partnered with the YMCA to offer its Blood Pressure Self-Monitoring Program.
Description
High blood pressure, also known as hypertension, affects almost 50% of all Americans (CDC). Blood pressure that is checked frequently and well-controlled helps decrease the risk of complications, including heart attack and stroke. In partnership with the YMCA, the Blood Pressure Self-Monitoring Program is a 16-week, evidence-based program that features personalized support and four nutrition education seminars. You will develop the habit of routinely self-monitoring your blood pressure and learn tips for maintaining your cardiovascular health. Individual weekly virtual or phone meetings will be scheduled with your Certified Healthy Heart Ambassador.

Seminar 1: February 26th, 2026
Seminar 2: March 26th, 2026
Seminar 3: April 23rd, 2026
Seminar 4: May 21st, 2026
Eligible participants will receive a free blood pressure monitor and cuff. In order to participate, you must meet the following requirements:
- 18 years old or older
- Diagnosis of high blood pressure or currently taking antihypertensive medication
- No cardiac event in the last year
- No diagnosis of atrial fibrillation or other arrhythmias
- No risk for lymphedema
Technology requirements: internet access and a computer, laptop, tablet, or smartphone with video camera, microphone, and speaker capabilities.
