Project LINK, Inc is the administrative arm of the Back-to-School Coalition of Hillsborough County. The Coalition consist of medical collaboratives that include BayCare, USF Health, Tampa General Hospital, Florida Dept of Health- Hillsborough, South University, Gannon University, and Hillsborough County Public Schools health services.
Our focus is to ensure school-aged students are provided with shot records transfers, routine vaccinations and/or school physical to meet enrollment requirements for Hillsborough County Public Schools.
Our clinics are held annually during the summer and families are required to register in advance. To be eligible to attend a health clinic, students must meet the following requirements:
Parent or legal guardian must accompany child at all times; Immunization record is required to receive shots and/or shot records transfers;
child must be uninsured, under-insured or have Medicaid coverage; No current private insurance.
No Sports, Head Start or Child Care services will be provided.
If your child is only receiving a physical, you don't need to bring his/her shot records.
Your child must be at least 5 years old and entering kindergarten this year to be eligible.
Please note, if your child attended a Hillsborough County Public or Charter School last year, he/she DOES NOT require another school physical this academic year for enrollment. The School District only requires 1 physical during a child’s K – 12 tenure.
If your child requires an Immunization or Shot Record Transfer, You MUST have a hard copy of his/her shot records with you, at the time of appointment. NO EXCEPTIONS!
f your child is transferring from a private school or coming from another State or Country and his/her immunization is up to date, but need shot records transferred to a Florida Form DH-680, you must bring his/her current shot records with you to process the transfer. You can also contact the Florida Department of Health (813) 307-8000 for this service or if need further assistance.
In light of the ongoing COVID-19 pandemic, The Back to School Coalition of Hillsborough County is continuing its preventative measures to reduce the spread of COVID-19 during the Back-2-School Health Clinics.
To protect the health of patients and staff, we are limiting the number of Immunizations and School Physicals offered, in addition to restricting the attendance to only 1 parent or guardian to escort child(ren) receiving medical services.
The COVID-19 VACCINE WILL BE AVAILABLE FOR CHILDREN 5 TO 18 YEARS OLD.
