Back 2 School Health Clinic
TGH is proud to partner with Project LINK to offer Back to School Health Clinics that provide Medicaid or uninsured families with access to NO COST routine immunizations, school physicals, and immunization records transfers.
Description
Our focus is to ensure school-aged students are provided with shot records transfers, routine vaccinations and/or school physical to meet enrollment requirements for Hillsborough County Public Schools.
Our clinics are held annually during the summer and families are required to register in advance. Walk-ins are not accepted. To be eligible to attend a health clinic, students must meet the following requirements:

Must be new to Hillsborough County Public or Charter Schools.

Must be uninsured, under-insured or have Medicaid coverage; No Private Insurance.

Enrolling in the 7th grade this year and need a routine immunization booster shot. (T-Dap)

Enrolling in Kindergarten this year, and must be 5 years or older on or before September 1, 2024.
Please note:
- If your child attended a Hillsborough County Public or Charter School last year, he/she DOES NOT require another school physical this academic year for enrollment. Hillsborough County School District only requires 1 physical during a child’s K – 12 tenure. Please see the child's primary care physician for an annual check-up.
- If your child requires an Immunization or Immunization Record Transfer/Update, You MUST upload a copy of the immunization record during registration and bring a hard copy of his/her shot records with you, at the time of appointment. No exceptions!
- If your child is transferring from a private school or coming from another State or Country and his/her immunization is up to date, but requires Immunization records transferred to a Florida Form DH-680, you must bring his/her current shot records with you to process the transfer.
