National Diabetes Prevention & Lifestyle Change Program
The Florida Department of Health – Hillsborough County and Tampa General Hospital have partnered together to provide the CDC’s National Diabetes Prevention Program which has proven to help people diagnosed with pre-diabetes reduce their risk for developing type 2 diabetes by participating in this evidence-based lifestyle change program.

The Centers for Disease Control and Prevention (CDC) estimates that more than 79 million Americans age 20 and older, have prediabetes. The majority of people living with prediabetes do not know they have it. A person with prediabetes has a blood sugar level higher than normal, but not high enough for a diagnosis of diabetes. Prediabetes is a serious health condition that increases the risk of developing type 2 diabetes, heart disease and stroke.
Am I at risk?
Many factors can increase your risk for prediabetes and type 2 diabetes. Risk factors can include:
- being 45 years of age or older
- being overweight
- having a family history of type 2 diabetes
- being physically active fewer than three times per week
- ever having diabetes while pregnant (gestational diabetes) or giving birth to a baby that weighed more than nine pounds
Your health matters! The Florida Department of Health – Hillsborough County and Tampa General Hospital have partnered together to provide the CDC’s National Diabetes Prevention Program which has proven to help people diagnosed with pre-diabetes reduce their risk for developing type 2 diabetes by participating in this evidence-based lifestyle change program. This program will teach you why it’s important to prevent diabetes, how to eat healthier, become more physically active and how to manage your weight.
This FREE program is offered over the course of a year with 16 weekly sessions, followed by bi-weekly sessions for the remainder of the program.
Who Can Participate in the Program?
If you have been referred by your doctor, have prediabetes, have been told by a doctor that you are at risk for developing type 2 diabetes, or have a body mass index (BMI) of 24 or higher, then you are eligible to participate in the National Diabetes Prevention & Lifestyle Change Program.
