Description
Chronic health diseases affect almost 1 in 2 American adults, negatively impacting their quality of life. Chronic health conditions include any on-going health issues such as anxiety, arthritis, balance, respiratory and heart disease. The Chronic Disease Self-Management Program is an online interactive group program with a series of 6 different sessions for people with any type of chronic diseases, their caregivers, concerned friends and family members.
Registration is required as seating is limited. Once registration closes, you will receive a link to attend this workshop.
Program dates: Tuesdays 4:00 —6:30 p.m.
April 15, 22, 29; May 6, 13, 20, 2025
Technology requirements: Internet access, computer, laptop, tablet or smartphone with video, microphone, and speaker capabilities.
